As more& more youngsters use earphones 
&headphones to video or voice chat & listen to music, damage from everyday loud noise has seen a marked increase.Youth today are at a higher risk of #hearingloss. But how can we protect from this,What are the danger signs? 
 By an Audiometer, we can present tones of various frequencies in various intensities or loudness, to test one's hearing. By having the patient raise their hand or press a button to indicate that they heard the sound, we can measure the softest sound that a patient can hear at each frequency in each ear. Usually, sound at 100 decibels will cause pain in the ears.
Loud noise can damage our hearing by Demaging, the hair cells in #Cochlea ,the hearing organ.The damage depends on the length of time the individual is exposed to loud sounds& the loudness of the sound itself.If the sound is too loud, one's hearing can be damaged in a second. But if it is not too loud, the duration of exposure will determine the degree of hearing loss. The permissible noise exposure time for sound at 85 decibels is 8hours,& it is 4 hours for 90 decibels ,2for95,1hr for95,half hr for100,15 minutes for 105db.Beyond this intensity ear should not be exposed unless protected by ear plug &/or Muff.
To avoid noise-induced hearing loss, shorten the listening time for loud sounds. It is OK if the volume is in the low and mid-range. However, it is suggested that people not listen to portable listening devices in public areas like on buses or the subway because these places are so noisy that they might turn the volume very high to a point that can be harmful to their hearing without noticing. Also more chances of accident.For ear care, wearing headphones or earphones will increase the risk of infection for people with sensitive skin.
If the sound is uncomfortably loud, it is usually an indicator of harmful levels.
For noise-induced hearing loss, depending on how bad the exposure is, there will be ringing in the ears, muffled hearing or aural fullness (a sensation of blockage in the ear) after noise exposure.It is advised that people stay away from loud noises to protect their hearing or help recover from noise-induced hearing loss. For mild damage, it usually takes 24 hours to recover. If there is no relief of symptoms after 24 hours, ENT intervention is needed. If the damage is severe, hearing loss could only be partially restored or even irreversible.

What is new drum repair surgery ?

Majority of patients coming with ear discharge or hole in ear drum or hearing loss when advised Tympanoplasty or Tympano-mastoidectomy ask What is new ear drum ? Tympanoplasty means exploration of middle ear ,removal of disease if there any and repair of ear drum &/or ossicles (3 tiny bones for hearing).New drum formation takes place after 3 wks or more, after we put temporalis fascia taken from behind &above pinna or perchondrium &/or cartilage as Graft.when disease demand only ,then mastiod bone(which is extension of middle ear cleft behind) is also drilled open or removed.Bones of hearing may be refashioned from existing ones if needed or may be cartilage or titanium etc prosthesis can be used to improve hearing.Successful operation means stoppage of discharge ,closure of hole and improvement in hearing .patient should not have complaints of persistent pain or itching hence preoperative nasal sinus and throat infections must be treated and pt must avoid cold exposure and infections.Head wash must be avoided for 3wks so also forceful blowing nose. however a valsalva exercise in absence of cold is beneficial.Microsurgery of ear today has very high success rate,done under local anaesthesia except in children and should not be feared. patient can go back to his/her town next day and resume work on day 3 rd.

sneezing...why morning?

A sneeze, is a semi-autonomous, convulsive expulsion of air from the lungs through nose &mouth, usually caused by foreign particles(dust, pollen/other tiny particles in air)irritating the nasal mucosa;often happens suddenly&without warning. Sneezing is linked to sudden exposure to bright light, sudden change (fall) in temperature, breeze of cold air, a particularly full stomach, or viral infection,&can lead to the spread of disease. Sneezing typically occurs when foreign particles or sufficient external stimulants pass through the nasal hairs to reach the nasal mucosa,triggers the release of histamines, which irritate the nerve cells in the nose, resulting in signals being sent to the brain to initiate the sneeze,viaTrigeminal nerve. Biological clocks - dictated by circadian rhythms - move in concert with the immune system, prompting timely sneezing bouts.Every time we go to sleep, our immune systems get a reset.Like the rest of our bodies,the system down shifts into a lower gear during this period during which we are fighting fewer outside invaders. Endogenous circadian rhythm in the immune system,generally tends to rev up in the later stages of sleep,but we do not notice until we wake.It looks like body has a sense of when it is about to wake up &greet the day-&all of the allergens that come with it.People who suffer from bouts of sneezing at exactly the same time every day,have problem in morning.Flu&viral inf,sinusitis,deviated septum,polyps,bleeding etc r common causes

Unusual case of drug side effect -‪#‎coatedtongue‬
‪#‎Azithromycin‬ -though a safe drug ,has this unusual presentation ,in a 30 yr old female having URI,with #coatedtongue,prominent taste buds,ulcer tongue and swelling lips.coated due to candida n lip swelling due to allergic reaction.gastric intolerance,colitis also common and should be avoided in liver demage cases.azithromycin is overused especially by ‪#‎physicians‬‪#‎generalpractioneers‬etc,although it is not the drug of choice in ‪#‎ENT‬ infections where co-amoxiclav or cephalosporins are preferred drug.Another issue is not often, explained to patient, to take it ‪#‎emptystomach‬ in morning and no beverage or breakfast for 45 minutes;although one can also advise to take tablet with no food/beverage 2 hr before and one hour after, tablet taking.let us think several times before prescribing to diminish over usage. ‪#‎Dosage‬ also should be 500mg twice daily in adult if at all it is used in ear,sinus n throat infections.however it has good result in ‪#‎gonococci‬,‪#‎chlamydia‬,‪#‎staphylococci‬ infections alone or in combination with other antibacterial.
